What You Should Bring to Your First Visit

To help us make the most of your time, we recommend bringing:

  • A photo ID
  • Your dental insurance card (if you have one)
  • A list of any medications or health conditions
  • Any recent dental records or X-rays you might have
  • Your completed new patient forms (you can also fill them out here)
